SLOBC Ride Incident Report

An SLOBC Ride Incident Report is Required for any Weekly, Monthly or Annual SLOBC Ride during which a participating member is injured in any way or level of severity. Use the following button to access the online Incident Report.

SLOBC Incident Report 2025

  • Timing: Submit an Incident Report within 72 hours. It is the rensponisbility of the Ride Leader to submit or ensure that a report is submitted.
  • Save Draft: Use the "Save Draft" button to be able to update/complete the form at a later date. You will be sent an email with a link to use when you are ready to complete the form.
  • Documentation: Use your phone (Notes, Voice Memos, Camera etc.) to document the incident and assist in completing the report.
  • Submit Form: When submitted, a copy of the form will be emailed to the member submitting the report, the member involved in the incident and, the SLOBC Board. All incident report information will be kept private.

Report Goals

The purpose of the SLOBC Incident Report is to understand the causes of injuries during our rides in order to reduce their frequency in the future.

  • Create a record of Incidents occurring during club rides
  • Document the categories and specific factors that contribute to Incidents
  • Document how Incidents are resolved
  • Improve safety practices to reduce future Incidents and improve their resolution
  • Keep the Board informed of Incidents
  • Minimize the effort required to create reports
  • Refine information gathered to support report goals

Incident App

The Incident Report App gives you direct connection to the SLOBC Incident Report as an icon on the home screen of your phone. Navigate to this page on your phone and then click on the following Incident Report App link.

Once the report opens, click on the phone's share icon. Then scroll down the list of share options and click on Add to Home Screen. Note: If a message appears before you click on the share icon, close the message and proceed.

You will be presented an option to edit the name of the app. Edit it to include only the year.

This will place the app icon on your phone that will open the SLOBC Incident Report when you click on it. Move the app to the desired screen or location.

It is recommended that all ride leaders setup the app on their phone.
