SLOBC Incident Report - Online

The SLOBC Incident Report - Online is intended for use by members to report incidents that occur on the club's Weekly, Monthly and Annual Rides. This report should not be used for the Wildflower and Lighthouse Century rides.

Reports are required for "Minor" injuries that result in bumps, bruises and/or road rash and for "Major" injuries that result in the rider abandoning the ride, or police or EMS being dispatched.

Notes

Multiple People: Create a separate report for each rider involved in the incident.

Submit Form: When submitted, a copy of the form will be emailed to the person submitting the report and the SLOBC Board. Our insurance requires that all slobc incident reports be kept on file in case a claim is made against the club or member.

Save Draft: Use the "Save Draft" button to be able to update/complete the form at a later date. You will be sent an email with a link to use when you are ready to complete the form.

Documentation: Use phone Notes, Voice Memos, Camera or other applications to document the incident. If anyone has an Anchor Point application, use it to record the incident location. Ask others to help. Complete the report within 72 hours.

Incident App

The Incident Report App gives you direct connection to the SLOBC Incident Report as an icon on the home screen of your phone. Navigate to this page on your phone and then click on the following Incident Report App link.

Once the report opens, click on the phone's share icon. Then scroll down the list of share options and click on Add to Home Screen. Note: If a message appears before you click on the share icon, close the message and proceed.

This will place the app icon on your phone that will open the SLOBC Incident Report when you click on it. Move the app to the desired screen.

It is recommended that all ride leaders setup the app on their phone.
